New Patient Paperwork

For Functional Medicine (aka Nutritional Medicine):
For those seeking help with autoimmune disorders or other metabolic conditions from the Functional Medicine side of the clinic, paperwork is not required before your first visit.  Each month, Dr. Sweeney hosts two free informational sessions designed to discuss what Functional Medicine is, and how it may help various conditions; we also invite you to schedule a one-to-one consultation with Dr. Sweeney in order to discuss your particular issues.  At the end of either of these sessions, Dr. Sweeney will answer any questions you may have about the program, and provide you with all of the necessary intake forms and questionnaires you'll need to get started on your journey.

For Advanced Neurological Chiropractic:
Initial Intake Form Packet (PDF) - all new chiropractic patients - this must be completed before your first visit.

Motor Vehicle Accident Form (PDF) - for all new patients who have EVER been involved in a major accident, OR ANY accident within the past 7 years, no matter how minor.

If you've been involved in more than one accident that meets these criteria, please download and complete one form for each accident.  These forms give us a good idea of what happened, how to approach treatment, and what sort of outcome we can expect.

For Medical and Integrative/Elective Massage Therapy:
Patients receiving Medical Massage Therapy as directed by the doctor under their treatment plan don't need to fill out additional massage therapy intake forms.  

Clients who would like to receive massage therapy at our clinic may schedule with our therapist, even if you're not under care of the doctors.  Please understand that as a doctor's office, we do things a little differently:

Massage Therapy Intake Forms (PDF) - these forms must be completed and submitted to our office before the we can schedule the first appointment.   This is to allow our therapist a chance to review your information so that she can tailor the visit to your needs; this also helps to minimize short-notice cancellation and missed appointments.
